Well it isn't really a piercing. I've seen piercings before, this is an actual "ball" under the skin.

No, it's not a piercing, but it's the exact same concept as something like a Frenum ladder.

It doesn't sound that appealing, and I have no way of knowing if it would make sex better since I have no experience with modified cocks penetrating my orifices.

As to your second question - who knows? Maybe anticipating getting out of jail? Maybe they're butt fucking each other and didn't want to admit it on camera.
